What happens in your Initial Assessment?

During your initial assessment we will aim to gain a thorough understanding of your reasons for seeking nutritional support and your concerns. We will conduct a thorough assessment which is important to ensure that subsequent consultations are relevant and helpful to you. We will then discuss your personalised nutritional support plan including treatment approaches and frequency of subsequent consults.

Subsequent Consultations

Subsequent consults allow for personalised support and counselling in which we progress you through concepts, problem solve and develop strategies to help you. Some people require longer support depending on their presentations and progression between appointments.

Personal Information

A client’s personal information is gathered and used for the purpose of providing nutritional services, which includes assessing, diagnosing and treating a client’s presenting concern/s. Examples of personal Information we collect include: names, date of birth, address, email address, phone, Medicare number, Health Fund details and personal health information. This personal Information is obtained in many ways including interviews, correspondence, by telephone and facsimile, by email, website and from third parties. We don’t guarantee website links or policy of authorised third parties. We may also use your personal Information for secondary purposes closely related to the primary purpose, in circumstances where you would reasonably expect such use or disclosure. When we collect personal Information we will, where appropriate and where possible, explain to you why we are collecting the information and how we plan to use it. The personal information is retained in order to document what happens during sessions and enables us to provide a relevant and informed nutritional service.

Confidentiality

We are members of Dietitians Australia (DA) and are committed to DA’s Code of Professional Conduct and Statement of Ethical Practice. We are committed to maintaining the confidentiality of your personal information by complying with the Privacy Act 1988 and the Australian Privacy Principles 2014.  In accordance to the Health Records Act 2001, all information collected is treated as confidential information. We use your information only for the purpose of providing you with quality care. In some circumstances, we will disclose selected information to other health providers if it directly relates to your care i.e. medical practitioner, specialist or other allied health practitioners.

 

Disclosure of Personal Information

Your Personal Information may be disclosed in a number of circumstances including the following:

– Third parties where you consent to the use or disclosure; and
– Where required or authorised by law.

In circumstances of liaising with a third party with regards to your care your prior approval will be obtained to:

– Provide initial and regular progress reports to your referring health professional or another health professional involved in your care.
– Discuss your care with another person.

Please note, for clients presenting with eating disorders and/or mental health concerns it is a requirement that we communicate with your GP and psychologist.

Access to your Personal Information

You may access the personal Information we hold about you and to update and/or correct it, subject to certain exceptions. If you wish to access your personal Information, please contact us in writing. Nutrition to live will not charge any fee for your access request but may charge an administrative fee for providing a copy of your personal Information. In order to protect your personal Information, we may require identification from you before releasing the requested information.

Maintaining the Quality of your Personal Information

It is an important to us that your personal Information is up to date. We will take reasonable steps to make sure that your personal Information is accurate, complete and up-to-date. If you find that the information we have is not up to date or is inaccurate, please advise us as soon as practicable so we can update our records and ensure we can continue to provide quality services to you.

Fees

Fees are payable at the time or just prior to the session via Visa, Mastercard or EFTPOS or direct bank transfer. We cannot accept Amex or cash. The fees apply to both face-to-face and virtual sessions (via Zoom). If an account remains unpaid within 5 working days, an administrative fee of $30 applies, plus an interest rate of 10% per annum calculated from the invoice date.

Fees represent payment for professional knowledge and experience. They also support treatment planning and work done outside the session to support the therapeutic relationship. We pride ourselves on being a premium nutrition service with a strong focus on providing a high standard of care and giving our clients the time they deserve. We are therefore unable to offer bulk billing services and there will be out-of-pocket expenses for your consultation after any relevant Medicare rebate.

Medicare Rebates

In order to receive a Medicare rebate your eligibility for a Chronic Disease Management Plan (formerly Enhanced Primary Care or EPC) will need to be determined by your GP. If you have a Chronic Disease Management Plan you will be entitled to a Medicare rebate from the consultation fee. Under Medicare you will then be eligible for up to 5 rebatable dietetic sessions per calendar year under the Care Plan. An Eating disorder plan allows for 20 visits per year.

Health Insurance Rebates

You may be able to claim a rebate for dietitian services from your private health fund. Private health fund rebates vary based on your level of cover and do not require a referral. We recommend that you check with your health fund directly for further information. You can use either Medicare or your private health insurance, but not both.

Invoices

Invoices will be provided to you by email which can then be taken to Medicare or your Private Health Insurer.

Cancellation Policy

When an appointment is scheduled, that time is dedicated to you and your treatment. If, for some reason, you need to cancel or reschedule an appointment, we require at least 24 business hours’ notice. Consultations cancelled without providing 24 business hours’ notice attract a 100% cancelation fee with the exception of emergencies. If you miss an appointment without notifying us will you be charges 100% of the consult fee. Charges incurred for cancellations without required notice and missed appointments are not claimable through Medicare or private health funds.

Third Parties

Where reasonable and practicable to do so, we will collect your Personal Information only from you. However, in some circumstances we may be provided with information by third parties. In such a case we will take reasonable steps to ensure that you are made aware of the information provided to us by the third party.
